ABA supervision notes for BCBAs (CPT 97155)

Write in Spanish, get a professional English ABA session note, ready for your billing platform.

A 97155 note documents protocol modification: what the BCBA observed, what was adjusted and why, and when it will be evaluated again. NoteCoach ABA builds it from what you mark and from the client's plan. Each note is checked against a documentation checklist for its CPT before you copy it.

What a 97155 note shows

  • Face-to-face observation of whether the protocol is working or needs adjustments
  • Active direction given to the RBT, when an RBT took part: modeling, correcting errors, training on a modified protocol, feedback, IOA recording
  • Adjustments to the components of the protocol, each with what changed and the observation behind it: treatment targets, treatment goals, observation and/or measurement, reinforcers, reinforcer delivery, prompts, instruction, materials, discriminative stimuli, contextual variables
  • QHP implementation of the protocol with the client
  • A re-evaluation timeline (3, 4, 5, 6, 8 or 10 sessions)

When no RBT took part, the note does not mention one. When an RBT's implementation is the problem, retraining comes before changing the protocol. The note never invents a fidelity percentage.

Also for BCBAs

The caregiver is identified by name and relationship; only the training techniques you tick (BST, modeling, role-play, verbal instruction, written handout, video) appear; the caregiver's report about practice outside sessions is used only when you enter it. Sessions with the client absent are written as role-play.

97151 / 97151-TS: assessment

One paragraph per clinical activity you tick (observation, interview, record review, assessment administration and scoring, report preparation, discussion with caregivers), with the assessments administered, scored or analyzed.
